Bloom Functional Medicine does not accept any type of health insurance, whether private or government funded plans including Medicare. We are happy to provide you with an itemized superbill with ICD-10 codes that you may use to file with your insurance company. You may be able to use your Health Savings Account (HSA) to help the cost of Bloom Functional Medicine’s fees, please check your individual plan. All costs will be transparent.

60 minute in-person appointment
Review of initial intake form and case preparation

Office visit with complete history and physical examination
Detailed written personalized plan which includes individualized recommendations for lifestyle modifications, including dietary,exercise, sleep, and exercise habits
Recommendations and ordering of laboratory tests
Welcome kit consisting of informational handouts, supplement organizer, and gratitude journal

Bloom Initial Consultation Follow Up
First appointment following the Bloom Initial Consultation
30 minute in-person or virtual appointment
Review lab work results
Discuss lifestyle modification implementation and strategies tailored plan with further lifestyle changes
Targeted supplement recommendations
Recommendation of advanced laboratory tests
Cost: $239
After the first follow up appointment:
Patients may choose future appointments based on their medical needs
We have found that early on in most patient's health journeys, appointments are required more frequently
As time goes on, the appointment intervals may be spread out a bit more

The number of visits required varies for each patient based on their individual health needs and goals.
BFM - Monthly Appointment Plan
Recommended for patients who have advanced testing completed and/or have multiple medical issues
30 minute appointment (in-person or virtual)
Appointment will consist of:
Review of chronic medical conditions
In depth review of medications & supplements
In depth review of advanced laboratory tests
Detailed recommendations for lifestyle modifications (dietary, sleep, stress, and exercise habits)
Detailed information and recommendations for targeted vitamins, minerals, and nutraceuticals
Adjustment of pharmaceutical medications if needed
Cost: $229/month for a minimum of 3 months
BFM - Individual Appointment
Recommended for patients who don’t need advanced testing completed or have less medical issues
30 minute appointment (in-person or virtual)
Appointment will consist of:
Review of chronic medical conditions
In depth review of medications & supplements
In depth review of advanced laboratory tests
Detailed recommendations for lifestyle modifications (dietary, sleep, stress, and exercise habits)

Additional Information
Bloom Functional Medicine uses encrypted, HIPAA compliant software (AcuityScheduling.com) for billing and appointment scheduling. Alternative payment methods are accepted (cash, check, PayPal, Venmo) on a case-by-case basis.
Many HSA and Flex accounts cover medical services such as Functional Medicine. Please check with your individual plan for details of coverage.
Bloom Functional Medicine will gladly provide you with a detailed invoice/superbill to submit to your insurance company. Please make sure to check whether your insurance company covers visits with “out of network” providers, which is what Bloom Functional Medicine is classified as.
Advanced laboratory tests are charged as a separate fee. You may submit your insurance information to the lab company directly to obtain insurance coverage for many of these tests. Always check with your insurance company for full information regarding coverage.
